Confidentiality
During the first visit, all clients are required to fill out a Client History
Form regarding their past and present health status. Any information shared on
this form or spoken verbally to the therapist is kept completely confidential.
Your private information collected for business purposes will not be given to
anyone, for any reason, unless directed to do so by you expressedly in writing.
Massage Room Policy
The client's modesty is always respected, and draping regulations established
by state law are strictly followed.
Any child under the age of 13 must have a parent or legal guardian present in
order to receive a massage. A child between the ages of 13 and 18 may receive
massage without a parent present if a signed consent form is provided.
Small children should not accompany a parent during a massage session.
Babysitting services are not provided. An older child is welcome to "tag along"
if they sit quietly in the waiting area.
Cancellation
A 24 hour notice is requested for cancellation of appointments.
Late
If a client is more than fifteen minutes late, they may be given a shorter
massage session, but charged the full session price. If the client is
excessively late, the appointment may be rescheduled, but will be handled as a
"no show" (see below).
No Show
If the client fails to cancel and does not show up for the scheduled
appointment, they will be assessed 50% of the cost of the missed session. This
fee will be added to the next scheduled appointment. Please keep in mind that a
"no show" results in loss of income for the therapist, who could have filled
your spot with another client.
NSF Check Policy
All checks returned from the bank as NSF (insufficient funds) will be held for
10 days and assessed a $25 NSF charge. Clients must pay the entire amount due
on the check held as NSF plus the $25 NSF charge to avoid collection/prosecution.
